Results of a Phase III study (N = 177) comparing Sativex, THC-predominant extract and placebo in intractable pain due to cancer unresponsive to opiates (Johnson and Potts 2005) demonstrated that Sativex produced highly statistically significant improvements in analgesia (Table 1), while the THC-predominant extract failed to produce statistical demarcation from placebo, suggesting the presence of CBD in the Sativex preparation was crucial to attain significant pain relief.
I suffer from osterarthritis, degenerative disc disease with so far one neck triple fusion of vertebrae, lower disc bulging discs, Class 3 Hip disease, fibromyalgia, just to name a few. Cannador® (IKF-Berlin) is a cannabis extract administered in oral capsules, with differing figures as to THC:CBD ratios (reviewed in (Russo and Guy 2006)), generally approximately 2:1. Two pharmacokinetic studies on possibly related material have been reported (Nadulski et al 2005a; Nadulski et al 2005b). In a Phase III RCT employing Cannador in spasticity in multiple sclerosis (MS) (CAMS) (Zajicek et al 2003) (Table 1), no improvement was noted in the Ashworth Scale, but benefit was observed in spasm-associated pain on subjective measures. Both Marinol and Cannador produced reductions in pain scores in long-term follow-up (Zajicek et al 2005). Cannador was assayed in postherpetic neuralgia in 65 subjects with no observed benefit (Ernst et al 2005) (Table 1), and in 30 post-operative pain subjects (CANPOP) without opiates, with slight benefits, but prominent psychoactive sequelae (Holdcroft et al 2006) (Table 1).

The Duquenois–Levine test is commonly used as a screening test in the field, but it cannot definitively confirm the presence of cannabis, as a large range of substances have been shown to give false positives. Despite this, it is common in the United States for prosecutors to seek plea bargains on the basis of positive D–L tests, claiming them to be conclusive, or even to seek conviction without the use of gas chromatography confirmation, which can only be done in the lab. In 2011, researchers at John Jay College of Criminal Justice reported that dietary zinc supplements can mask the presence of THC and other drugs in urine. However, a 2013 study conducted by researchers at the University of Utah School of Medicine refute the possibility of self-administered zinc producing false-negative urine drug tests.

Cannabis use and psychotic symptoms and disorders are associated in the general population (see, for example, Degenhardt and Hall, 2001; Tien and Anthony, 1990) and in clinical samples of patients with schizophrenia (Mueser et al., 1992; Warner et al., 1994; Hambrecht and Hafner, 1996). The major contending hypotheses to explain the association have been: (i) that cannabis use precipitates schizophrenia in persons who are otherwise vulnerable; (ii) cannabis use is a form of self-medication for schizophrenia; and (iii) that the association arises from uncontrolled residual confounding by variables that predict an increased risk of cannabis use and of schizophrenia (Macleod et al., 2004).

Mississippi Mississippi allows the use of cannabis extract,s like oil or resin that contains more than 15% CBD and less than 0.5% THC. The United States Federal Government does not define ‘hemp’ exactly, but they do define ‘industrial hemp’ to be any part of a cannabis plant, whether growing or not, that is used solely for industrial purposes (fiber and seed) with a THC concentration of no more than 0.3 percent when dried. In contrast, Hemp.com defines it as “the fiber and seed part of the Cannabis Sativa L. plant, opposed to the flower part of the plant which is ‘legally considered’ marijuana.”
Marijuana or marihuana (herbal cannabis), consists of the dried flowers and subtending leaves and stems of the female Cannabis plant. This is the most widely consumed form, containing 3% to 20% THC, with reports of up-to 33% THC. This is the stock material from which all other preparations are derived. Although herbal cannabis and industrial hemp derive from the same species and contain the psychoactive component (THC), they are distinct strains with unique biochemical compositions and uses. With President Trump signing off on the Agricultural Improvement Act of 2018 (aka the 2018 Farm Bill) last month, the federal government now fully recognizes hemp as a legal agricultural product. But while many reports are claiming that this means that cannabidiol (CBD) is also legal, that’s not quite correct. With a lot of misinformation flying around, and contradictions between state and federal laws, things are admittedly somewhat confusing. Let’s try to sort things out by answering some questions about hemp, CBD, and what has recently changed in federal law.
Some research suggests that CBD also eases pain by engaging glycerin receptors – which are part of the central nervous system and are located in different parts of the brain and spinal cord. If a person has chronic pain, inflammatory factors sometimes disable these receptors, leading to higher sensation of pain, but it’s possible to reactivate them using CBD.
Research suggests that CBD may exert some of its pharmacological action through its inhibition of fatty acid amide hydrolase (FAAH), which may in turn increase the levels of endocannabinoids, such as anandamide, produced by the body. It has also been speculated that some of the metabolites of CBD have pharmacological effects that contribute to the biological activity of CBD.
Pure CBD cannabis oil derived from hemp contains only trace amounts of THC. Hemp is grown from specific cannabis varieties that naturally possess higher levels of CBD. These hemp stalks and hemp seeds produce organic hemp oil that is naturally rich in cannabidiol. Some genetic varieties of hemp contain higher concentrations of pure CBD than others.
Doctors advise pregnant women not to use any drugs because they might harm the growing fetus. Although one animal study has linked marijuana use to loss of the fetus very early in pregnancy, two studies in humans found no association between marijuana use and early pregnancy loss. More research is necessary to fully understand the effects of marijuana use on pregnancy.
Buying CBD oil online is problematical. A study from the University of Pennsylvania published in 2017 found that nearly 70 percent of products sold online do not contain the concentration of the oil listed on the label. The researchers bought and analyzed 84 products from 31 different companies and found that more than 42 percent were under-labeled, meaning that they contained more CBD than indicated. Another 26 percent were over-labeled, with less CBD than indicated. The researchers wrote that while studies haven’t shown that too much CBD can be harmful, products containing too much or too little may not give purchasers the effects they seek. In addition, the team reported that a number of products analyzed contained a significant amount of THC, which can cause undesired effects.
This article will attempt to present information concerning cannabinoid mechanisms of analgesia, review randomized clinical trials (RCTs) of available and emerging cannabinoid agents, and address the many thorny issues that have arisen with clinical usage of herbal cannabis itself (“medical marijuana”). An effort will be made to place the issues in context and suggest rational approaches that may mitigate concerns and indicate how standardized pharmaceutical cannabinoids may offer a welcome addition to the pharmacotherapeutic armamentarium in chronic pain treatment.

In 2014, the South Carolina legislature passed S 1035/H 4803, also known as “Julian’s Law.” The law creates an exemption for the possession and use of CBD from the criminal definition of marijuana in limited circumstances. Only patients with severe forms of seizure disorders are eligible for legal protections after the patient obtains a recommendation for CBD oil from a physician.

Years passed, and more studies rolled out with medically beneficial findings regarding cannabis until 2009 when Steep Hill Laboratory in Oakland, California, tested cannabis samples provided by Harborside Health Center to discover that a handful of cultivars contained more CBD than THC.
